How BPHM works
One workflow. Five coordinated steps.
From installation to a documented outcome, here is what actually happens and who is responsible at each stage.
- 01
Scope and install
We agree together on which rooms or clients take part, the installation plan, who our designated contact is on your side, and the milestones where we review results. Our team mounts the sensors. There is no facility Wi-Fi to configure and nothing for a resident to wear.
- 02
Learn personal patterns
Each person's baseline develops over approximately two weeks of usable readings. Timing varies by individual and by data availability. Nobody is compared to a population average, only to their own usual range.
- 03
Review changes
Sensors collect continuously. Our clinical team reviews resident and client trends daily against protocols signed by our Medical Director. Continuous collection and daily review are two different things: we do not present this as around-the-clock staffed observation.
- 04
Coordinate the next step
When a reviewed change warrants action, we contact your designated care contact with a specific request, and involve the treating provider where that is appropriate. Testing and treatment decisions belong to the provider.
- 05
Document and follow up
Every item we raise is closed with a documented outcome: confirmed, ruled out, or resolved. We keep watching the trend afterwards so a change that comes back does not have to be rediscovered.
Who does what
Monitoring supports usual care. It does not replace your staff, your clinical judgement, or the treating provider, and it is not an emergency response service.
BPHM
- Install and maintain the monitoring hardware
- Review personal trends daily against physician-signed protocols
- Raise specific, already reviewed follow-up requests with your designated contact
- Document the outcome of every item we raise
Your community or agency staff
- Name a designated contact and a route for after-hours questions
- Carry out the requested check or observation and tell us what was found
- Follow your normal urgent and emergency procedures, which monitoring does not replace
- Notify families and providers according to your own policies
The treating provider
- Interpret the clinical picture for their patient
- Order any testing and decide on treatment
- Direct the plan of care and any change to it
What we review in a pilot
At each milestone we sit down with your own data. We do not bring benchmarks from other buildings.
- Usable monitoring coverage: how much good data we actually captured per person
- Reviewed changes: how many trend changes met protocol and were reviewed
- Follow-up actions: what we asked for and what your team did
- Documented outcomes: how each item closed, confirmed, ruled out, or resolved
- Staff experience: whether the requests were clear, useful and reasonable in volume
